Employee killed when thrown from basket of mobile platform

Employee #1 was unloading a JLG powered mobile platform from a trailer. The front wheels of the platform slipped, causing the rear wheels to hit the ground with such impact that the telescoping boom sprung up, throwing Employee #1 into the air. He landed on the guardrail of the basket. His weight pushed down the basket, which struck against the bed of the trailer. The boom then sprang up again, and Employee #1 was thrown from the basket a second time. He again fell on the guardrail of the basket, and then tumbled approximately 8 feet onto the pavement. Employee #1 was taken to the hospital, where he died of head and chest injuries.
