EMPLOYEE KILLED WHEN HE FELL THROUGH A SKYLIGHT

A NINE MEMBER CREW WAS INSTALLING A NEW ROOF ON A LIBRARY BUILDING. THE GROUND-TO-EAVE HEIGHT WAS ABOUT 14 FEET. THE ROOF PERIMETER WAS GUARDED BY A 4 FOOT, 6 INCH HIGH PARAPET WALL. EMPLOYEE #1 WAS LOADING ROLLS OF FELT ONTO A WOODEN PALLET, LOCATED NEAR A 40 INCH DIAMETER SKYLIGHT. THE SKYLIGHT WAS COVERED BY A 40 INCH PLEXIGLASS DOME. EMPLOYEE #1 FINISHED COVERING THE PALLET WITH A PLASTIC SHEET AND STEPPED BACK TOWARDS THE SKYLIGHT. HIS FOOT STRUCK AN 8 INCH HIGH LIP WHICH SURROUNDED THE SKYLIGHT. HE TRIPPED AND FELL BACKWARDS THROUGH THE PLEXIGLASS SKYLIGHT (.063 THICK) TO THE LIBRARY FLOOR. THE ACCIDENT OCCURRED ON SEPTEMBER 17, 1986, AT ABOUT 5:30 PM. EMPLOYEE #1 DIED ON SEPTEMBER 21ST.
