Investigation Summary
AT THE TIME OF THE ACCIDENT THE EMPLOYEES WERE MOVING BUNDLES OF PIPE. EMPLOYEE #1 AND ANOTHER EMPLOYEE WOULD HOOK TWO WIRE ROPE SLINGS TO THE ENDS OF A BUNDLE OF 6 INCH PIPE. THE BUNDLES WERE BEING PICKED FROM THE TOP OF A 10 FOOT STACK OF PIPES. THE ROPE SLINGS WERE CONNECTED TO THE LOAD LINE OF AN OVERHEAD CRANE. HOOKS WERE POSITIONED AT EACH END OF THE PIPE BUNDLE. THE BUNDLES WERE LIFTED BY A WHITING 25 TON OVERHEAD CRANE. THE ACCIDENT HAPPENED AFTER ONE EMPLOYEE HOOKED UP HIS END OF THE SLING AND LEFT THE STACK TO GET A DRINK OF WATER. THE OTHER EMPLOYEE (EMPLOYEE #1) HOOKED UP HIS SIDE OF THE SLING AND GAVE THE CRANE OPERATOR, LOCATED IN THE CRANE CAB, THE "UP" SIGNAL. AS THE LOAD WAS LIFTED, ONE END BECAME HUNG UP ON OTHER PIPE BUNDLES IN THE STACK. WHEN THE STACK PULLED LOOSE IT SWUNG BACK IN THE DIRECTION OF EMPLOYEE #1. THE LOAD STRUCK THE BUNDLES EMPLOYEE #1 STANDING ON AND DISLODGED THEM. THE BUNDLES ROLLED OFF THE STACK AND STRUCK AND KILLED EMPLOYEE #1, WHO HAD FALLEN WITH THE BUNDLES.
