Employee killed in fall from unguarded trailer

Employee #1, the president, had positioned the top of a truck trailer against a one-story truck terminal that was being erected. He was on top of the trailer, installing double-sided tape to a structural member to which insulation blankets were to be adhered. The trailer served as a work platform and as fall protection for coworkers who were working on the sloped roof. The sides of the trailer, however, were not guarded and did not prevent Employee #1 from falling. He fell about 13 1/2 feet, presumably striking the dock plate with his feet first, and was then flung another 5 feet to the ground. Employee #1 was found dead in a pool of blood from his head. No one witnessed the fall.
