Investigation Summary
At approximately 4:00 p.m. on May 24, 1993, Employee #1 entered an area beneath a trim press to monitor and/or repair ejector rods on the press platen. The employee failed to shut the press down before entering it. His head was caught between a cylinder bracket and a gear box. Employee #1 was pronounced dead at a local hospital from massive head injuries.
