Employee suffers head injuries when forklift turns over

At approximately 3:00 p.m. on August 16, 1993, Employee #1 was hoisted about 18 feet above the ground on a personnel platform attached to the forks of a Hi Lift truck to reach a security scanning camera he had come to repair. The camera was mounted on a light pole in the company's parking lot. Employee #1 had completed repairs on one side of the camera and was being transported, with the forks elevated, around the light pole to the reverse side, when the forklift tipped over on its side. Employee #1 fell to the ground along with the platform. He was hospitalized for head injuries.
