Investigation Summary
At approximately 9:30 a.m. on June 20, 1994, Employees #1, #2, #3, and #4 were doing concrete repair work on an overpass abutment. Employee #3 was working from a portable scaffold, while Employees #1, #2, and #4 were working in and around a pickup truck and compressor. A semi-trailer flatbed loaded with construction supplies came loose from the pickup truck that was towing it and crashed into the work site, striking the compressor and truck and injuring the Employees #1 through #4. All were hospitalized. Employees #1, #2, and #4 sustained various contusions, abrasions, and fractures. Employee #4 sustained injuries that required his right leg to be amputated.
