Employee falls 30 ft and dies

Employee #1 and a coworker were carrying 20 ft long needle beams on a plywood subfloor just beneath the roof over an operating paper machine at a paper mill. Although Employee #1 was wearing a full body harness, and static lines were provided, Employee #1 had not tied off. The coworker, who was in the lead, felt the beam being dropped behind him as he rounded a section of duct work. When he turned around, Employee #1 was not visible. Employee #1 had lost his balance, and fallen 30 ft through a 48 in. by 53 in. hole to a concrete mezzanine. He died.
