Employee killed in fall from roof

Employee #1 was installing insulation panels on a large flat roof. The employer was using a warning line that was 17 feet from the roof edge. No one witnessed the actual accident but for some reason, Employee #1 moved to the unprotected roof edge and fell 23 feet to a concrete floor. An emergency rescue unit arrived in two minutes and pronounced him dead.
