Investigation Summary
At 2:15 p.m. on February 9, 2015 an employee working as a maintenance contractor for VDOT when he sustained a 277 volt electrical current which killed him. The employee and coworker were working on light poles that were not operational on Northbound I95 at exit 169A Springfield, VA. One coworker was at the disconnect that supplied power to the junction boxes and pole lights that is located approximately one minute away, behind a dealership inside panel 199. The employee was prepping a conductor to splice together at the junction box by pole # 011. The coworker at the disconnect, hooked up a shot box used to find faults in conductors, and proceeded to turn the power back on to the junction box that the employee was working in. Another coworker was writing a report in his truck when he looked over and saw the employee laying over the junction box and not responsive. The employee and coworker at the disconnect were not in eye sight of one another, and they were using cell phones to communicate with each other.
