EMPLOYEE #1 WAS USING A 12 FOOT EXTENSION LADDER TO ADJUST SOME TEMPERATURE CONTROLS WHICH WERE LOCATED IN THE SKYLIGHTS IN THE ATRIUM ABOVE THE PLANTER. FOR UNKNOWN REASONS HE FELL AND WAS KILLED.
