Investigation Summary
On January 9, 2015, Employees #1, #2, and #3, and Coworker #1, employed by a construction contractor, were working at a multi-employer commercial building construction site. They were applying stucco to the side of the building from the third and second buck work platforms of a tubular welded frame scaffold. Coworker #1, a supervisor, was operating a forklift and raising materials up to the employees working on the scaffold. The forklift ran into the scaffold, causing it to collapse. The three employees fell, a fall height of 21 feet. Emergency services were called, and the employees were transported to the hospital. Employee #1 was admitted and treated for bilateral leg and ankle fractures. Employee #2 was admitted and treated for a skull fracture. Employee #3 did not require hospitalization.
