Investigation Summary
Employee #1, who had been on the job 41 days, was assigned to work with two other employees who had more experience. That day, Employee #1 was given the job of cleaning out the photograde rail tankers. Employee #1 and an operator strung out the water hose to be used in the tank. Employee #1 was wearing a full face MSA respirator with 2 MSA chem cartridges--6MD ammonia. Employee #1 climbed into the tank to start washing it down while the operator waited on the ground. After about 15 to 20 minutes, the operator called to Employee #1 but received no response. The operator then climbed up to the platform and looked into the tank, calling for Employee #1. The operator observed Employee #1 lying in the back part of the tank and went for help. About 10 to 20 minutes later, emergency personnel arrived and started rescue procedures. When Employee #1 was taken out, no vital signs were observed. Employee #1 was pronounced dead on arrival at the hospital.
