Investigation Summary
At approximately 9:50 a.m. on October 31, 1989, Employee #1 was attempting to straighten a 1/2 in. steel banding that was stretched across 250 ft wide structural steel bar joists approximately 32 ft above the concrete floor. No fall protection of any kind was provided. Employee #1 had walked from the metal deck roof area onto the bar joist to reach the steel banding that had snagged and would not shake loose. He reached over and picked up the bands and was tugging on them when they came loose and he fell backward. He fell to the concrete, landing on his feet in a standing position, then falling on his left side. His hard hat fell off at some point during the fall. He died of traumatic massive head injuries the next day at 5:25 p.m.
