Employee killed in fall from roof to frozen ground

Employee #1 was assisting a friend who was building a new home. They were installing roof sheeting and covering on the garage roof, which had a pitch of 9:12 and an eave-to-ground measurement of approximately 15 ft 10 in. Both men were using a nailed 2 by 4 as a foot brace; no other fall protection was in use. For no apparent reason Employee #1 lost his balance/footing and slid off the roof. He landed on the frozen ground and suffered a compound fracture of the left leg and massive head injuries. Employee #1 was transported to a local hospital by Air Care helicopter and underwent emergency surgery. He did not regain consciousness and died as a result of his head injuries.
