EMPLOYEE FALLS FROM ELEVATION WHEN GUARDRAILS REMOVED

SIX EMPLOYEES WERE ENGAGED IN REMOVING AN OUT-OF-SERVICE EXHAUST FAN AND RELATED EQUIPMENT THAT HAD ORIGINALLY BEEN INSTALLED IN A DUST COLLECTION SYSTEM. THE EQUIPMENT WAS LOCATED ON THE SECOND LEVEL APPROXIMATELY 26 FEET ABOVE GROUND LEVEL. THE FAN AND RELATED EQUIPMENT WERE TO BE REMOVED AND SENT TO HOUSTON FOR RENOVATION AND INSTALLATION IN ANOTHER LOCATION. ON DECEMBER 11, 1991, EMPLOYEE #1 USED AN OXYGEN/ACETYLENE TORCH TO CUT THE UNIT LOOSE FROM ITS MOUNTINGS AND DUCT WORK. THE GUARD RAILINGS WERE ALSO CUT OFF TO ACCOMMODATE THE REMOVING OF THE FAN UNIT. THE FAN AND HOUSING WERE ESTIMATED TO WEIGH APPROXIMATELY 700 TO 800 POUNDS. A CASE FRONT END LOADER WAS PLACED ON THE GROUND, A WIRE ROPE WAS ATTACHED TO THE LOADER AND RUN THROUGH A PULLEY THAT WAS ATTACHED TO A STRUCTURE BEAM, AND THE ROPE WAS ATTACHED TO THE FAN HOUSING. AN EMPLOYEE WAS STANDING AT A VANTAGE POINT TO THE SOUTH TO GIVE INSTRUCTIONS TO ANOTHER EMPLOYEE WHO WAS OPERATING THE LOADER. TWO OTHER EMPLOYEES WERE HOLDING 2 TAG LINES SO THAT THE FAN WOULD NOT SWING UNCONTROLLABLY WHEN IT WAS LIFTED OFF THE FLOOR. THE LOADER OPERATOR HAD BACKED UP AND TIGHTENED THE ROPE WHEN EMPLOYEE #1 WHO WAS STANDING BETWEEN THE FAN AND THE OUTSIDE EDGE OF THE PLATFORM, SAID "STOP" AND BENT OVER AT THE BASE OF THE FAN UNIT. AS EMPLOYEE #1 BEGAN TO RISE FROM A STOOP, THE FAN SHOOK VERY SLIGHTLY. IT APPEARED THAT EMPLOYEE #1 THOUGHT THE FAN WAS GOING TO FALL; HE LOST HIS FOOTING AND FELL OVER THE SIDE OF THE PLATFORM TO HIS DEATH.
