Investigation Summary
Employee #1 was standing on the truckbed of a truck-mounted crane while a waxed cardboard container was being removed from the roof of a building. As the operator swung the container off the roof, the side of the container broke open, showering the employee with parts weighing less than 1 lb each. He was killed.
