EMPLOYEE KILLED WHEN PINNED BETWEEN DOCK AND TRAILER

THE DRIVER OF A TRACTOR TRAILER BACKED HIS RIG UP TO A LOADING DOCK AND GOT OUT TO INSTALL WHEEL CHOCKS. HE DECIDED THAT THE TRAILER WAS NOT POSITIONED EXACTLY AS HE WANTED IT SO HE GOT BACK IN THE TRUCK AND PULLED THE RIG FORWARD. HE WAS UNAWARE THAT EMPLOYEE #1 HAD ALREADY BEGUN LOADING THE TRAILER THE INDUSTRIAL TRUCK WHICH EMPLOYEE #1 WAS OPERATING WAS HALF ON THE TRAILER WHEN IT MOVED FORWARD. THE FORKLIFT SLIPPED DOWN BETWEEN THE TRAILER AND THE DOCK PLATE. EMPLOYEE #1 WAS PINNED (WITH THE STEERING WHEEL PRESSING ON HIS CHEST) BETWEEN THE FORKLIFT AND THE DOCK. THE EMPLOYEE WAS TRANSPORTED TO ST. VINCENT'S HOSPITAL WHERE HE DIED THREE HOURS LATER.
