Investigation Summary
A TWO-PERSON CREW WAS RECONNECTING A DAMAGED OIL WELL POWER SUPPLY LINE. THE SWITCH FOR EACH PHASE OF THE 12.47-KILOVOLT CIRCUIT WAS OPENED AT A SWITCHING STATION LOCATED ABOUT 1000 FEET AWAY FROM THE DAMAGE. AFTER THE CIRCUIT WAS OPENED, THE POWER LINE WORKER AND HIS GROUND HELPER LOOKED AT AN OIL WELL ON THE CIRCUIT TO SEE IF IT WAS STILL OPERATING. (THEY DID NOT TEST OR GROUND THE LINE.) ALTHOUGH THE OIL WELL WAS NOT OPERATING, ONE OF THE THREE PHASES OF THE CIRCUIT REMAINED ENERGIZED DUE TO A DEFECTIVE SWITCH ARM. AFTER MAKING THE VISUAL CHECK, THE POWER LINE WORKER CLIMBED A POLE AT THE DAMAGE SITE AND CONTACTED THE ENERGIZED CENTER PHASE CONDUCTOR. THE LINE WORKER WAS ELECTROCUTED.
