EMPLOYEE CRUSHED TO DEATH BY FALLING STEEL SCRAP

AT APPROXIMATELY 3:25 PM ON MARCH 4, 1991, EMPLOYEE #1 WAS USING A CUTTING TORCH TO CUT A HIGHWAY GRADER MACHINE INTO SCRAP. EMPLOYEE #1 COMPLETED CUTTING ONE OF THE PIVOT ARM ATTACHMENTS ON THE HIGHWAY GRADER IN HALF. ONCE IT WAS CUT THROUGH, THE TOP HALF OF THE PIVOT ARM FELL ON EMPLOYEE #1, CRUSHING HIM AGAINST THE FRAME OF THE MACHINE. THE EMPLOYEE DIED OF HEAD INJURIES.
