EMPLOYEE KILLED DUE TO FRACTURED NECK FROM FALL

A stock attendant was washing a portable tank over a grated 6-ft-deep pit. The pit was partially filled with waste water and waste chemicals from the washing process. The spigot of the tank became tightly wedged in one of the 1-in. by 4-in. grate openings. As the employee tried to dislodge the spigot, the grate he was standing became displaced and the employee fell into the small opening between two grates. As he fell through the space created between the displaced grate and the adjacent grate, his neck was caught and fractured. He was killed and was later found.
