Employee killed in fall through roof insulation

Employee #1 was a member of a five-person crew installing the roof of a steel aircraft building. He and a coworker were putting down 6 ft wide fiberglass insulating material between the structural skeleton and metal sheeting and then modular strips to permanently secure the insulation and as spacers for the metal sheeting.. Employee #1 was holding up a piece of C-channel to allow the coworker to install a modular strip when he fell backward through the insulation they had just installed. He fell 25 ft 5 in. onto a concrete floor and was killed. C-channel is iron used for weighing down the insulation on windy days, prior to installing the metal sheeting.
