Investigation Summary
At approximately 10:30 a.m. on November 19, 1992, Employee #1 was using a metal probe to search for the stub of the main sanitary sewer line in a 43 ft long by 2 ft 6 in. to 4 ft wide by 4 to 7 ft deep trench. The trench walls were vertical and unshored. When Employee #1 turned north to get ready to exit the trench, the west bank caved in and knocked him against the east bank. Employee #1 was buried up to his neck and sustained seven fractured ribs and a fractured shoulder blade.
