Employee killed when struck by uncoiler arm in chest

At approximately 9:15 a.m. on May 7, 1992, Employee #1 was picking up scrap metal material around his work station. The employee then picked up the material from the floor near the uncoiler of the Gate Channel Roll Forming Line. He was struck on his chest by the extended arm of the uncoiler. Employee #1 sustained a crushing injury to his chest and was killed.
