Investigation Summary
At 11:00 a.m. on February 19, 1991, Employee #1 was securing/nailing plywood sheeting to trusses on the 6:12 pitch roof of a residential building under construction. The ground-to-eave height was approximately 24 ft. A single planked carpenter's scaffold (without railings) was installed below the fascia. A catch platform was not in use. Employee #1 fell from the roof onto the frozen ground and died from the injuries he sustained. Weather was not a factor. No alternative fall protection was in use. The employee never fired the nailer before the accident.
