Employee killed when run over by coke battery quench car

Employee #1, a maintenance repair employee, was working with another maintenance employee attempting to install a bell warning device on the axle of a coke battery quench car between shifts. Neither employee locked out the main power, which was 250 volts DC, because they thought it was only going to take 10 to 15 minutes and the next shift didn't start for about 30 to 40 minutes. They ran into problems with bolts that were too short. Once they retrieved bolts that were the right size, both employees were under the quench car installing the bell on the end opposite the electric locomotive. A quench car operator from the next shift got in the locomotive without seeing or hearing the two maintenance employees and moved the unit toward the coke battery. He ran over Employee #1; the coworker was able to scramble out. Employee #1 died.
