Investigation Summary
AN OVERHEAD POWER LINE WAS BEING UPGRADED FROM 2400 VOLTS DELTA TO 7200 VOLTS. A NEW CROSSARM HAD BEEN INSTALLED ON THE UTILITY POLE, AND A NEW TRANSFORMER HAD BEEN PLACED ON THE POLE AS WELL. AN EMPLOYEE WORKING FROM A PITTMAN HYDRAULIC AERIAL LIFT WAS MOVING A PHASE CONDUCTOR ONTO THE NEW PIN INSULATOR ON THE CROSSARM. THE EMPLOYEE, WHO WAS NOT WEARING RUBBER INSULATING GLOVES, WAS ELECTROCUTED WHEN HE CONTACTED THE OVERHEAD LINE.
