Employee killed in fall from ladder

At approximately 2:30 p.m. on November 11, 1992, Employee #1 was approximately 22 ft high, working from an extension ladder. The upper end of the ladder slipped to the left, causing Employee #1 to fall and strike his head on the concrete floor. He was killed.
