Employee fractures leg in trench cave-in

At approximately 10:00 a.m. on March 30, 1993, Employee #1 was preparing to set a manhole basin in a 12 ft long by 10 ft wide by 8 ft deep trench. The walls were almost vertical and neither sloped nor shored. Employee #1 had just graded the trench and was climbing over the 8 in. water line when the west bank caved in, pinning him against the water pipe. Employee #1 was hospitalized with a fractured left leg.
