Investigation Summary
Employee #1 left his work position and was running (semi-jogging) west in the warehouse. At the same time, a forklift was backing up in the aisle to the north of the warehouse to pick up a loaded pallet. Employee #1 had his head turned northeast toward the work station and was still running when he ran into the left side of the forklift. He fell backward, hitting his head against the concrete floor. The forklift driver had tried to turn north toward the dryer to avoid hitting Employee #1, but the employee did not see the forklift. Preliminary reports from the medical examiner indicated that Employee #1 died of head injuries.
