EMPLOYEE #1 WAS WALKING BACKWARDS ON A ROOF, STRETCHING OUT AN EXTENSION CORD HE HAD JUST PLUGGED IN. HE WALKED RIGHT OFF THE EDGE OF THE ROOF AND FELL 40 FEET TO HIS DEATH.
