Employee killed in fall from floor edge

Employee #1 was cutting off metal end plates from concrete beams using a cutting torch on the corner of the 12th floor of a building under construction. After completing the task, he relocated away from the floor edge to clean the torch. Later, for unknown reasons or causes, and unseen by other workers, he returned to the floor edge and fell, landing on the cement floor of a balcony that jutted out from the 10th floor. He sustained injuries from which he died. The 12th floor edge, where he had been working, was equipped with taut cable guardrails, which were still intact after the accident. The employee's task could have been easily performed from behind the guardrails. His specific safety instructions were to remain behind the guardrails at all times. No cause for the fall has yet been determined.
