Investigation Summary
Employee #1 was reaching into a scissor lift for no apparent reason when his right arm became trapped between the table top and an I-beam that was part of the frame for the adjacent machinery. His head was smashed between the scissor lift bed and the I-beam, killing him. The clearance between the lift bed and the beam was 4 inches.
