Employee killed in 20 ft fall from steel support beam

Employee #1 and coworkers were erecting a wood chip bin at a sawmill. He was on a steel support beam reaching for a container of bolts when he fell about 20 ft, fracturing his head and neck. He died in the hospital three days after the fall, as a result of complications from surgery. No fall protection was being used.
