Employee falls from scaffold and injured

On September 9, 2003, an employee was working on a scaffold approximately 2 ft wide, 5 ft long and 6 ft high. The scaffold platform was approximately 5.5 ft from the floor. The employee stood up from a kneeling position with his weight at or near the edge of the scaffold. The scaffold became unstable and tipped over causing the employee to fall and strike his head and back on the concrete floor. He was hospitalized and treated for concussion.
