EMPLOYEE KILLED IN FALL FROM FORKLIFT PLATFORM

On March 13, 1990, employees were installing an automated storage system consisting of two 3-ft-9.125-in. by 119-ft metal racks, 31 ft 10 in. in height. An unsecured 48 in. by 89 in. wooden platform was placed on the forks of the Clarklift. Employee #1, along with 15 aluminum containers, was elevated to the height of 20-25 ft. Employee #1 fell head down and was killed when placing the aluminum containers in the openings of the metal racks. The platform and two of the four remaining containers followed his fall. Employee #1 was not wearing personal protective equipment.
