Employee dies after striking head on driveway

At 1:30 p.m. on or about December 20, 1995, Employee #1 was apparently either walking around his truck or climbing the truck's fixed ladder when he fell and struck the back of his head on the driveway. No one witnessed the accident. Employee #1 was lying in the back of the truck. His legs and feet were pointing to the truck's ladder. His head was lying on the snow- and ice-covered ground. Employee #1 was pronounced dead at 9:50 a.m. on December 28, 1995.
