Employee killed when crushed by core machine

Employee #1 was installing a manifold on the tilt mechanism of a model CB-26-SA auto core machine, serial #114-CB26SA-AC. The power source was on and the machine inadvertently activated, crushing Employee #1's chest between the tilt mechanism and the frame of the machine. Employee #1 was killed.
