Investigation Summary
Employee #1, a pipe layer, was working alone at the bottom of a 22 ft deep trench. An 8 ft deep trench box was located in the excavation elevated approximately 8 feet above the trench bottom. A portion of an unshored wall, below the bottom of the trench box, collapsed. Employee #1 was buried under 3 ft of clay and was killed.
