Employee dies after fall from ladder

On November 10, 1994, Employee #1 was climbing a ladder with roof shingles in his arm. As he was going up the ladder, he fell 10 to 15 feet, striking his head. He was taken to the hospital at 9:40 a.m. and died on November 14, 1994, as a result of the fall.
