Investigation Summary
At approximately 7:00 a.m. on August 2, 1990, Employee #1 removed 37 sheets of plywood from a stack that was standing on end and placed them on a pallet, leaving a remaining stack of 47, 4 ft by 8 ft by 3/8 in. sheets of plywood. This stack fell, pinning him against the plywood that he had placed on the pallet. Employee #1 was crushed and died.
