CRUSHED BY OVERTURNING STOPGATE

TWO ELECTRIC UTILITY EMPLOYEES WERE USING A CHAIN FALL ON AN OVERHEAD MONORAIL TO MOVE A STOPGATE. THE STOPGATE, WHICH WAS 6 FEET, 9 INCHES HIGH, 11 FEET, 4 INCHES WIDE, AND 10 INCHES DEEP AND WHICH WEIGHED ABOUT 2000 POUNDS, WAS USED TO STOP THE FLOW OF WATER AT THE INTAKE ALONG THE RIVER. THE EMPLOYEES SET THE STOPGATE DOWN ON THE WALKWAY AND UNHOOKED THE CHAIN FALL. THEN AN AERIAL LIFT WAS MOVED INTO POSITION TO HOOK THE STOPGATE TO PLACE IT INTO ITS FINAL POSITION. AS THE AERIAL LIFT WAS MOVED OVER THE FREESTANDING STOPGATE, THE HOOK SUSPENDED FROM THE BOOM OF THE LIFT HIT THE STOPGATE AND KNOCKED IT OVER. IT FELL ONTO ONE OF THE TWO EMPLOYEES AND PINNED HIM AGAINST A GUARDRAIL. THE EMPLOYEE SUFFERED INTERNAL INJURIES CAUSING HIS DEATH.
