Investigation Summary
Employee #1 was unloading an aerial lift from a flat bed trailer onto a ramp. Employee #1 was maneuvering the lift from the employee basket. The ramp was not level with the trailer bed. This caused the boom and basket to bounce and snapped the employee back in the basket resulting in fatal injuries.
