Investigation Summary
Employee #1 was preparing to take a corn moisture sample from a load of corn in a tractor trailer. The Volvo-White tractor and Hawkeye hopper trailer were on the scales. The driver moved the tractor trailer and Employee #1 fell and was run over and crushed by the vehicle's passenger side dual wheels. The tractor and trailer had tandem dual axles.
