Investigation Summary
At approximately 5:35 p.m. on August 26, 1988, Employee #1 was told by his foreman, the plant superintendent, to move a seven-piece steel coil weighing 5,000 lb with a Towmotor forklift truck to the banding area. The foreman then left the employee to work alone. After Employee #1 lifted the cold form, oil lubricated steel coils off the floor, he apparently got off the forklift truck and walked to the front of the load to see if it was properly centered. The employee had left the load engaging means at a downward angle and the steel coils slid off the engaging means and struck him in the head, crushing his head. Employee #1 was killed. Apparently the load was not properly secured; lift truck malfunction was not a cause. Employee #1 was authorized to ride the forklift truck, but did not receive forklift operator training from the company.
