Employee struck and killed by falling 2 by 4

Employee #1 was emptying scraps from a scrap bin attached to the forks of a Buda forklift truck. He was working by himself and no coworkers were in the area. The bin fell and pinned the employee under a 2 by 4 running across the top of the open end of the scrap bin. The 2 by 4 struck the employee along the right side of his head. According to the coroner, Employee #1 died of multiple head and chest injuries.
