Investigation Summary
At approximately 11:00 a.m. on November 13, 1990, Employee #1 was repairing the roof of Carlson Plywood. He started on the north side, then proceeded to caulk screws on the south side of the roof at the fourth skylight. The employee was bending over and caulking the last screw, which was 6 to 8 inches from the edge, when he fell approximately 25 ft to the ground between the railroad tracks. He died.
