Investigation Summary
At approximately 12:55 p.m. on May 21, 1992, Employee #1 was standing at the rear of a flatbed truck guiding a load onto the truck. The load was being hoisted by a 1970 Link Belt truck crane (model #HC48A-SN347506)with a 20 ton capacity. The load being lifted weighed 3 tons. The two cables lifting the boom came loose from the cable anchors, causing the boom to fall onto the flatbed truck and strike the employee on the head. He was knocked to the ground and killed. The cable anchor failed because the cables were improperly assembled to the cable anchors.
