Investigation Summary
At approximately 7:48 a.m. on February 11, 1991, Employee #1 was working on a steeply sloped roof (greater than 4:12). The distance from the edge of the roof to the ground was 19 ft. Employee #1 complained to his brother that he was short of breath, but before his brother could come to his aid, he fell from the roof to the ground. He struck an air conditioning unit and was pronounced dead on arrival at the hospital.
