AN ELECTRIC UTILITY EMPLOYEE WAS INSTRUCTED BY HIS FOREMAN TO CLIMB A LADDER AND TIE IT OFF. WHEN THE EMPLOYEE ASCENDED THE LADDER, HIS LEFT HAND CONTACTED AN ENERGIZED BUS WHILE HIS HEAD WAS TOUCHING A GROUNDED BUS. THE EMPLOYEE WAS ELECTROCUTED.
